Intake Overview

How to Begin Therapy at HEROES

Start your journey in just three steps.

Step 1: Contact Us

Call or text (616) 460-3955. We’ll answer any questions and determine whether HEROES is the right fit for your child’s needs.

Step 2: Complete Intake Forms

We’ll email you digital intake forms. These include your child’s health history, developmental background, caregiver information, and any referral details. You can fill them out from your phone or computer at your convenience.

Step 3: Schedule the Evaluation

Once your forms are submitted, we’ll schedule your child’s in-person evaluation. We’ll also send reminders and preparation tips so you feel confident walking in.
